Health Coach – providing personalized lifestyle support, helping patients to increase activity levels, improve eating habits and maintain the long-term changes they need to improve their health and well-being. We can refer you for a 1:1 appointment or a group session.

Social Prescribing Link Workers aka Social Prescribers or Wellbeing Practitioners – providing holistic and personalized support to individuals, their families and carers, helping them take control of their wellbeing, live independently and improve health outcomes. Please let Reception know if you wish to be referred.

Other services supporting our practice

District Nursing Team – The Liscard team of community nurses provide nursing care in the community for housebound patients.

Health Visitors – based at Victoria Central Health Centre, they give advice on health care and development for babies and children under the age of five. Please call 0151 514 0219 for more information on days & times.
